Home-Based Care

Hope Hospice provides home-based care and services for individuals in St. Clair County. We believe that for many people with a life-threatening illness, the home is the best place to get care and maintain a quality of life. All of our home-based services are tailored to our patient’s individual needs and governed by a care plan developed by our interdisciplinary team with input from the patient and their family.

Pain/Symptom Control

At Hope Hospice, our goal is to alleviate pain for those with advanced illnesses. We provide compassionate pain management services, including medication management, to minimize physical discomfort and improve quality of life.

Comfort Measures

Our comfort measures are designed to provide physical comfort, reducing stress and inflammation that can worsen the pain. We offer a variety of comfort measures, from simple bathing and providing clean linen to therapeutic massages and more.

Medication/Equipment

At Hope Hospice, we provide medical supplies and equipment related to a patient’s hospice and palliative care when needed, including oxygen and medication.

Medical Management

Our hospice and palliative care services are offered by qualified healthcare providers who are supervised by Hope Hospice’s Medical Director. We provide support and guidance for the primary physician and other healthcare providers who work with our patients.

Nurse Practitioner

Hope Hospice Nurse Practitioners are an integral part of our interdisciplinary team. Our NPs provide education and guidance to our patients and families and help them understand their plans of care, medications, and therapies.

Primary Physician Care

Hope Hospice works with our patient’s primary physicians to ensure they border the best quality care during their time in hospice and that their wishes are respected. We provide comprehensive care planning, progress assessment, and care coordination throughout their journey with us.

Volunteer Services

Hope Hospice staff volunteers to provide companionship, respite for caregivers, and practical assistance in the home. Our volunteers provide physical and emotional support to our patients and families.

Home Health Aide

Our certified home health aides assist in a variety of ways, including assistance with activities of daily living (ADLs), personal care, assistance with meals, and light housekeeping. Our home health aides are trained and certified to provide the highest level of care for our patients.

Grief/Support Counseling

Grief counseling for our patients, family members, and caregivers is available before and after a patient’s death. Our counselors help individuals and families to understand the emotional and spiritual feelings that may arise at the end of life and after a loved one has passed.

Nursing

Hope Hospice qualified nurses coordinate and provide hands-on care to our patients. Our nurses provide symptom management, pain control, reassurance, and comfort during difficult times.

Chaplain Services

Hope Hospice recognizes the spiritual needs of our patients. We offer spiritual counsel and religious services as requested.

Social Work Assistance

Hope Hospice provides an individual social worker to every patient and family. Our social workers also visit regularly to assess the home situation, answer questions and provide emotional support during difficult times.

Determining the Level of Care

At Hope Hospice, we understand that each individual's care plan is unique and goes beyond medical treatment. Our interdisciplinary team includes professionals and paraprofessionals trained in their fields and one common goal: to provide the highest quality of care to our patients.
